Special Circumstances for Refunds (Certificate I-IV) Application Form

This application form is for Certificate I, Certificate II, Certificate III and Certificate IV students.

Please read the instructions for Applying for Special Circumstances Refunds (Certificate I-IV) before competing this form.

Section A: Student Details

Special Circumstances for Refunds (Certificate I-IV) Application Form

You may only apply for a Special Circumstances Refund if you withdraw from your unit of study after 28 days of the confirmed course commencement. You cannot apply for a refund for a unit of study that you have completed. Applications must be submitted within 12 months after the date of your request to withdraw. Refer to the Applying for Special Circumstances Refunds (Certificate I-IV) instructions for further information and required supporting documentation. Complete this form and attach supporting evidence. Melbourne Polytechnic will respond to your application within 15 working days of receipt. Please note it may take longer where additional information may be required to assess your application.
